The journal has been published by the Institute of History of the Polish Academy of Sciences since 1957 and was established by the community of Poznań and Warsaw medievalists, and was edited in both Poznań and Warsaw for many years. The first editorial committee included Aleksander Gieysztor and Gerard Labuda with Brygida Kürbis as secretary, who from vol. 2 (1958) became a member of the committee. For many years – from vol. 2 (1958) to vol. 31 (1987) – Ryszard Walczak (died in 1989) was the secretary of the editorial committee. The double volume 32–33 for 1990 was still edited by him, while the post of deputy secretary was held by Danuta Zydorek. Later, the secretary of the editorial committee was Tomasz Jasiński (vol. 34 for 1993), then Danuta Zydorek (vol. 35 for 1994 and vol. 36 for 1997).

In 1969 (from vol. 14) Ireneusz Ihnatowicz joined the editorial committee. Volumes 19 and 20 for 1974 and 1976 were prepared by the committee with Emanuel Rostworowski as a member. In 1976, (from vol. 21) Ryszard Kiersnowski joined the committee (to vol. 32 of 1990) and, in 1990, Maria Koczerska, Stefan K. Kuczyński and Józef Szymański followed suit.

Since 2000 (vol. 37), the journal has been published in Warsaw. The editorial committee for that time was initially composed of Zbigniew Dalewski, Maria Koczerska (editor), Stefan K. Kuczyński (editor) and Marek Słoń (secretary). In 2001 (vol. 39), it was extended to include Maria Starnawska (vol. 47) and, in 2009, Piotr Węcowski and Anna Salina (secretary), Andrzej Rachuba (editor) in 2011 (vol. 49), and Marek D. Kowalski in 2012 (vol. 50). Marta Piber-Zbieranowska was the journal’s secretary in 2010 (vol. 48) and continued to hold this position until 2019. Rafał Rutkowski was the journal’s editorial assistant in 2013–2022.

Currently, the function of editor-in-chief is performed jointly by Piotr Węcowski (WH UW) and Andrzej Rachuba (IH PAN).
